• How do I set up a new customer account?

  If you are a new customer and would like to set up an account to order blood components or samples for research or education, please complete and sign the Request for Research Products form and fax it to Kim Doeden, Research Coordinator, at 651-332-7001.

• What do I need to do to receive research products prior to testing?

  If you would like to receive blood products prior to testing, please complete and sign the Authorization for Untested Products form and fax it to Kim Doeden, Research Coordinator, at 651-332-7001. Positive (or reactive) test results for blood components will be reported to you in about 3 business days. Test results for samples will not be reported to you.
